*Minority Artist: For the purpose of this database and directory, a minority individual is defined as a Maryland resident who is a member of one or more of the following five groups. Please check each of the groups of which you are a member. (Individual only)
African American Native American
Asian/East Indian Pacific Islander
Hispanic or Latino Origin  

For the purpose of this database and directory, a minority organization is defined as a not-for-profit organization incorporated in Maryland which meets at least one of the following criteria. Please check each of the criteria your organization meets.(Organization only)
The staff of your organization is at least 51% minority.
The board of your organization is at least 51% minority.
The audiences your organization serves are at least 51% minority.
The artists your organization presents or serves are at least 51% minority.

Option A: I authorize MSAC to publish any or all of the information I have furnished in this questionnaire in a minority arts directory to be distributed to the general public. I understand that for purpose of publication MSAC reserves the right to edit the written description I have provided. I also understand that the information I have furnished will be kept in a MSAC database for the purpose of responding to inquiries regarding minority artists and/or organizations.
 
Option B: I authorize MSAC to maintain the information I have furnished as part of a MSAC database for the purpose of responding to inquiries regarding minority artists or organizations. I do not authorize publication of this information in a minority arts directory to be distributed to the general public.
